Using your QR code at a HICAPS provider

After your QR code is scanned by the HICAPS terminal, you will be asked to sign a declaration, which is located at the bottom of the transaction slip. Please read the transaction information carefully before signing the declaration, and check the following:

• That the patient ID number listed matches the number on the card and is the person that received the service
• What services are being claimed
• What ACA Health paid and
• The amount outstanding for you to pay

You should also retain a copy of the transaction for your records. Once your claim is processed, you can then settle any remaining balance while at your appointment.
